We are excited to announce our Save the Date for the virtual 2021 North Country Leadership Summit, with the AHI Leadership Summit joining forces with the state’s premier telehealth conference, brought to you by the North Country Telehealth Partnership (a collaboration of Fort Drum Regional Health Planning Organization and Adirondack Health Institute). This virtual one-week learning symposium will bring together leaders from New York’s North Country and beyond and is an opportunity to learn from experts on both telehealth, population health, and advancing whole-person care.

The Call for Presenters for the 2021 North Country Leadership Summit is now open. As our region and beyond moves toward a post-pandemic world, we want to highlight presentations that align with the conference theme of rebuilding and renewal. We are seeking engaging proposals in the following topic areas.

 

  • Sustainable Telehealth Programs, Tactics, and Techniques in a Post-Pandemic World
  • Cross Sector Collaboration Improving Social Determinants of Health in Rural Regions
  • Planning for a Post-Pandemic World in Health Care Systems and Facilities
  • Stories from the Frontlines: Innovative Ways to Reach Communities
  • Leadership Strategies for Employee Wellness and Workplace Retention During and Post COVID-19

Rural Health Champions

The Adirondack Rural Health Network (ARHN), a program of AHI, is seeking nominations for Rural Health Champions of the Year to recognize individuals who have made significant contributions to the health of rural residents in the region. Nominations are being accepted through Friday, August 6, 2021.

Champions will be announced at the 2021 North Country Leadership Summit, on September 15th. Rural Health Champions are individuals, such as health care providers, patient advocates, public health officials, human services personnel, and community-based organization program staff, who demonstrate leadership and provide exceptional care and service to rural community members.

Nominations will be accepted for individuals in the seven-county ARHN service region: Clinton, Essex, Franklin, Fulton, Hamilton, Warren and Washington.

Telehealth Innovator Awards

The North Country Telehealth Partnership (NCTP) is seeking nominations for the annual Telehealth Innovator Awards. These awards will be given to an individual (or individuals if more than one person contributed to successful deployment) that have advanced virtual care in the face of adversity amidst a global pandemic, which has led to improved health outcomes in the community, and/or improved quality of life. This can include telehealth deployed via videoconferencing, store-and-forward data or images, remote patient monitoring, or mHealth (mobile health) applications.

Contributions toward telehealth excellence and growth, including program expansion, patient impact, and innovation in new or emerging telehealth programs and models will be considered.

Nominations will be accepted for individuals in the 11-county NCTP service region: Clinton, Essex, Franklin, Fulton, Hamilton, Jefferson, Lewis, Saratoga, St. Lawrence, Warren, and Washington.
