AHI Health Home Program Redesignated for Three Years by NYS DOH

The AHI Health Home program has received a three-year redesignation from the New York State Department of Health. Health Home is a care management service that helps connect community and social supports with health care, and assists individuals who need help connecting with local health care providers, specialists, and other resources in the community.

The AHI Health Home received redesignation after an on-site performance survey conducted by the NYS DOH in conjunction with the AIDS Institute, Office of Mental Health, Office of Alcoholism and Substance Abuse Services, and the National Center on Addiction and Substance Abuse.

Among other accomplishments highlighted in the redesignation letter, it was noted by the NYS DOH that the AHI Health Home “successfully worked with care managers, providers and managed care plans to increase Health Home enrollment from 986 in January 2013 to 3,002 in January 2016.”
