Thursday, January 22nd, 9:00 am  – 6:00 pm.  Enroll in health insurance such as Medicaid, Child Health Plus, or a qualified health plan through the NY State of Health Marketplace. What to bring:

• Social Security numbers and birth dates for everyone in the household
• Income documentation (tax return and/or pay stubs)
• Email address and password (we can help set one up for you)
• Current insurance (please bring your insurance card)

• Citizenship documentation
• Your online NY Gov ID and password
