NYS DOH has released the Vital Access Provider (VAP) Exception application.

If you are a Medicaid provider who needs an exception to the safety net rule you need to submit the application to DOH by 10/14/2014. There are instructions as well as the application at the link above. DOH asked if you have questions about this application for you to e-mail [email protected] with your facility name and “DSRIP VAP Exception Question” in the subject line.
