Help New York fill in the gaps in the health care system – smarter spending, more coordination and a healthier population.

New York State is moving full-speed ahead to create a stronger and more affordable health care system. Don’t get left behind!

A $100 million State Innovation Model grant has been awarded to the New York State Department of Health (NYSDOH) to fund a new Advanced Primary Care (APC) model, value-based payments and resources for improving primary care practices throughout the State.

Participation and buy-in from health plans, employers and regional health collaboratives is crucial to the success of this initiative.

The Northeast Business Group on Health (NEBGH), in partnership with the NYSDOH, is committed to working with employers, payers and other stakeholders to develop a sustainable primary care model for which we are willing to invest.

This past July, NEBGH conducted eight listening sessions throughout the state and received invaluable feedback that will play an important role in the design of the Advanced Primary Care (APC) model and the work that is presently underway.

We’re coming back to share the latest developments and ask for additional feedback.
