Providers from the Northern Pod Medical Home participated in AHI’s May 14 presentation in Plattsburgh. Lottie Jameson, VP Regional Health Planning and Development, gave an AHI overview and spoke about community health services; Medical Home Initiatives Director, Bob Cawley, reviewed Health Systems Transformation Initiatives, including DSRIP; Annette Parisi, Health Home Manager, wrapped up the presentation with a description of Health Home and its regional impact.
