The North Country Population Health Improvement Program (NCPHIP) serves a six-county North Country region – Clinton, Essex, Franklin, Hamilton, Warren and Washington – in a data-driven effort to bring together a variety of stakeholders that impact, or are impacted by, health and health care issues.
