Clinicians and practice managers from the Australian Association of Practice Management (AAPM), as part of a Canada/U.S. study tour, convened with Adirondack Medical Home Initiative (AMHI) partners for an information exchange event on May 7. Hosted by Adirondack Health Institute (AHI), the clinicians and managers visited the North Country to gain an understanding of the region’s integrated primary health care delivery system.
The clinical visits and educational presentations held at University of Vermont Health Network – Champlain Valley Physicians Hospital were the only U.S. stops on the tour. The Australian clinicians and practice managers were drawn to upstate New York because of their interest in AMHI’s innovative Patient Centered Medical Home model for the delivery of health care services that strengthen the role of primary care.
