The Patient-Centered Primary Care Collaborative has published its annual report which highlights recently-published clinical, quality, and financial outcomes of patient-centered medical home (PCMH) initiatives across the U.S. As a provider of a variety of central services that support the Adirondack Region Medical Home Pilot, we found this to be an illuminating and encouraging report of findings with regard to the number of primary care practices that have embraced the PCMH model of care.
