Interim Steering Committee and Regional Health Care Innovation Leadership Teams (RHITs) structure and membership
Healthcare leaders from across the nine-county region began rolling up their sleeves the first week of November to tackle projects that aim to reduce preventable hospital admissions by 25% over a five year period. The AHI Interim Steering Committee Members are working with the leadership of the DSRIP (Delivery System Reform Incentive Payment) Regional Health Care Innovation Leadership Teams [RHIT]. New to the Malone/Lake Placid/Saranac Lake team is co-leader Patti Hammond, COO, from Adirondack Health. The five regional teams are focused on planning the behavioral health and chronic condition projects. AHI staff and consultants will do the initial planning on projects addressing an integrated delivery system, the medical village, population health and patient centered medical home, then work closely with the Teams to finalize those plans. The RHITs include representation from across the care continuum, and work collaboratively to address the needs in a subset of the nine-county geography that constitutes a NOCN (Naturally Occurring Care Network).
