Organizational Transformation

AHI Organizational Transformation programs and projects are necessary to help partner organizations continually improve through effective fundamental change to an organization’s structure and culture.

An investment of federal resources to assist primary care practices in transforming to a highly-integrated, team-based model relevant to advanced primary care. Through a vetted set of milestones and transformation support, practices can achieve an ideal blend of consistency, flexibility, goal-setting, leadership, optimized workflow, and improved outcomes consistent with Value-based Payment (VBP) arrangements.
Cultural competency and health literacy initiatives promote health equity and reduce health disparities by increasing access to health care services and improving the quality of care provided. AHI has launched numerous CCHL training workshops to help partners work with individuals they serve to ensure they receive information in a clear and concise manner.
Rural regions can face many challenges in delivering health care, including barriers to patient travel and provider access. Telehealth and telemedicine are the delivery of clinical health care services through real-time video transmission to provide greater access to specialty and primary care, or remote patient monitoring in the home.

Community Support Services

Our Community Support Services programs support community members as we work toward the goal of improving population health, transforming health care delivery, and eliminating health disparities.

Enrollment Assistance Services and Education (EASE)

Our EASE program manages health insurance enrollment and community awareness services for the Federal Affordable Care Act implementation.

Health Home Care Management

AHI Health Home Care Management provides enhanced coordination of medical and behavioral health care by linking community and social supports with health care for high-risk Medicaid members.

Community Health Access To Addiction and Mental Healthcare Project (CHAMP)

The CHAMP program is designed to help individuals and their families get access to needed substance use disorder and mental health treatment services and get the most from their health insurance benefits.

Community Assessment and Prevention

Provides resources and infrastructure to promote the state’s Triple Aim of improving regional health care, population health and lowering health care costs.

Workforce

AHI supports the workforce activities of our partners by providing tools to help with training, recruitment, and retention efforts. One such resource is the HealthStream Learning Management System, an online learning management system that is a leading provider of workforce, patient experience, and provider solutions for the health care industry. HealthStream has partnered with top content partners to provide expert thought leadership and professional development courses for health care employees.
