The AHI Health Home was named March’s Health Home of the Month by New York Health Homes Learning Collaborative Online Community, a centralized communication and networking forum for New York’s 32 health homes. AHI Health Home exclusively covers six rural counties (Clinton, Essex, Franklin, Hamilton, Warren, Washington), and portions of two counties (Saratoga, St. Lawrence).
“I was proud to highlight AHI Health Home’s unique collaboration style. Our Health Home service providers really care about their patients, said Annette Parisi, Health Home manager.
“They keep an eye on the goal of effective care management, which means improving outcomes while decreasing costs. As a lead Health Home, AHI tries to remove barriers so care managers can spend time doing what they do best: providing compassionate and effective care coordination.”
AHI is a NY state-designated lead Health Home. Health Homes place an emphasis on linking community and social supports with health care and providing enhanced coordination of medical and behavioral health care.
