More than 50 DSRIP partners attended a Population Health Management (PHM) Learning Event in Lake Placid on Thursday, June 25. AHI’s Colleen Florio, VP, and Bob Cawley, Director, Health System Transformation, presented background information on the DSRIP initiative.
Two guests shared different perspectives/approaches to PHM:
- Corey M. Zeigler, MBA, CPHIMS, CSCS, Chief Information Officer, FDRHPO for North Country Initiative (NCI);
- Leah K. Fullem, MHCDS, Director of Informatics, OneCareVermont.
Mukesh Kakkar from KPMG introduced the two presenters illustrating Care Management and Reporting Analytics use cases.
