We’re kicking off the NYS DOH Population health Improvement Program (PHIP) for the North Country.
To be successful over the initial two year period, we need partners and stakeholders interested in collaboration.
Let’s have a conversation about our six county region …

- What community resources/regional infrastructures are needed to build the foundation for a healthier future?
