While the majority of Medicaid enrollees are relatively healthy, there is a growing number of beneficiaries who have complex medical, behavioral, and long-term care needs and present significant challenges to health care providers.
AHI, a New York State-designated lead Health Home, assists providers in caring for high-risk Medicaid patients with complex medical conditions.
A Health Home is not a physical location, but rather a care management service that focuses on linking community and social supports with health care and providing enhanced coordination of medical and behavioral health services. A care manager oversees and coordinates a client’s care, ensuring they receive all of the services necessary to stay healthy.
The AHI Health Home is a partnership of primary care providers, hospitals, behavioral health and community-based organizations. It serves all ages and covers all of Clinton, Essex, Franklin, Hamilton, St. Lawrence, Warren, Washington, and parts of Saratoga County.
