Health Home Development Funds in the amount of $1.227 million were recently awarded to AHI’s Health Home. Additional quarterly payments will be made through 2016. Care Management agencies will be involved in determining which of the following initiatives (one or more) will be funded: member engagement/promotion; workforce training/retraining; health information technology and clinical connectivity; joint governance technical assistance.
