In ongoing efforts to develop goals and strategies to promote the Triple Aim of better care, better population health, and lower health care costs in the Adirondack region, the North Country Population Health Improvement Program (NCPHIP) program will hold a stakeholder meeting to plan for the region’s 2016 population health improvement efforts.
The meeting agenda will include:
- A demonstration of the HealthyADK website and how it will support our efforts;
- A review of the recent HealthyADK and Population Health Improvement awareness campaign;
- A group discussion of our region’s health-related priorities and potential activities to address those priorities in 2016.
