Regional Health Disparities

Our six-county region of the North Country (Clinton, Essex, Franklin, Hamilton, Warren and Washington counties) offers a unique set of opportunities and challenges as we work together to build a healthy future.

Some of these challenges include:

  • Low rates of college education
  • Low household income
  • High unemployment rates
  • High disability rates
  • High rates of poverty

These social determinants affect health care

  • Smoking and obesity rates are high;
  • Mental health and substance abuse affect more than 30 percent of the Medicaid population.

Supporting the Region

AHI Programs support the region through collaborative planning and improvement efforts.
