AHI is hosting its 9th Annual Summit, Population Health in the Rural Landscape, from 8:00 a.m.—4:00 p.m., Thursday, September 21, at the Lake Placid Conference Center.
The Summit attracts upwards of 250 attendees annually, including health care providers, administrators, and representatives from community-based organizations, the business sector, and government.
Amy Boutwell, MD, MPP, President of Collaborative Healthcare Strategies, will deliver the keynote presentation, Designing and Delivering Whole-Person Transitional Care. A panel will then discuss DSRIP: Strategies in Action, sharing successes, challenges and lessons learned on regional DSRIP initiatives. Breakout session topics will include:
- Tackling Social Determinants of Health
- Community Members Leading Change
- Partnering to Improve Health
The cost to attend is $25/person and includes breakfast, lunch and refreshment breaks.
